DISCLOSURE STATEMENT
CONSUMER CREDIT FILE RIGHTS UNDER STATE AND FEDERAL LAW

You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor any credit repair company or credit repair organization has the right to have accurate, current, and verifiable information removed from your credit report. The credit bureau must remove accurate, negative information from you report only if it is over 7 years old. Bankruptcy information can be reported for 10 years.

You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a rental dwelling because of information in your credit report within the preceding 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is inaccurate information in your credit report due to fraud.

You have a right to sue a credit repair organization that violates the Credit Repair Organization Act. This law prohibits deceptive practices by credit repair organizations.

You have the right to cancel your contract with any credit repair organization for any reason within 5 days from the date you signed it.

Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may occur.

You may, on your own, notify a credit bureau in writing that you dispute the accuracy of information in your credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete information. The credit bureau may not charge any fee concerning an error should be given to the credit bureau.

If the credit bureau's reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau, to be kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about disputed informtion with any report it issues about you.

The Federal Trade Commission regulates credit bureaus and credit repair organizations.

Cancellation Notice

Month To Month Agreement and Limited Power of Attorney

MONTH TO MONTH AGREEMENT & LIMITED POWER OF ATTORNEY

As a Client of Frank Carrasco Company "the firm" you "the client" agree to the following:

a. Discuss your situation and your desired outcome with "the firm" Specialist.
b. Acquire or help in acquiring your free credit reports from www.annualcreditreport.com, if you already acquired credit reports you "the client" are responsible for the cost of acquiring your credit reports.
c. Notify "the firm" of items you "the client" identifies as inaccurate, misleading, or unverifiable.
d. You agree to pay "the firm" the initial Set-up Fee by the 5th day from the date you have agreed to initiate the Month to Month Agreement & Limited Power of Attorney. The Set-up Fee is non-refundable. The monthly service is processing your requests and disputes to correct the data supplied on the credit reports with the collection agencies, creditors, and credit bureaus. Once the monthly payments have been paid, they are non-refundable, because service is fully performed prior to the assessment of charges. This Month to Month Agreement & Limited Power of Attorney renews every month when payment is made.
e. The monthly service is considered fully performed when one item is processed with a collection agency, creditor, and/or credit bureau.
f. The Set-Up service is considered fully performed after contacted by "the firm" Specialist and discussed accounts to be processed.
g. We are a data processing financial solutions company.
h. We are not a credit repair, credit fix, and/or credit restoration company because they are misleading.
i. To send via mail, fax, phone (when applicable), or email all credit reports and/or correspondence from the collection agencies, creditors, and/or credit bureaus within 5 days of receipt. (Mail recommended)
j. Print a copy of this Month to Month Agreement & Limited Power of Attorney for your records along with the federal disclosures.
k. Grant the Limited Power of Attorney to "the firm" as detailed below.
l. Grant permission for "the firm" to withdraw funds from credit card (when applicable).
m. Only communicate with the collection agencies, creditors and/or credit bureaus through "the firm".
n. You intentionally sought out "the firm" to perform the data processing financial solutions.
o. After your account setup payment is made you will be able to log in on the home page to view status of your disputes and view other added services.
p. Frank Carrasco Company (WhatTheFrank.com) Reserves the right to refuse service to anyone for any reason. 
q. Frank Carrasco Company (WhatTheFrank.com) Reserves the right to cancel any members service for any reason.
r. A single membership is $48.00 Account Setup / $28.00 Monthly Service. 
s. A couple membership is $72.00 Account Setup / $42.00 Monthly Service.

FREE SERVICE AGREEMENT
If you have signed up and where referred by a Mortgage Broker or Realty Company that is signed up with our Credit Mend Monthly Service. You will have Member status and benefit from our service for free as long as the Mortgage Broker or Realty Company pays their Credit Mend Monthly Service Fee. If the Mortgage Broker or Realty Company cancel their service you will be serviced for one more month from their cancellation date. You will have the opportunity to continue service for the monthly service rate of the Single or Couple Sign Up, whichever is applicable without paying a account setup fee. Please understand that the agreement is still effective with or with you not paying anything for the service. Your rights are still applicable. By allowing yourself to be referred you also allow the Mortgage Broker and/or the Realty Company to Sign you up if needed. Please note that the referring Mortgage Broker or Realty Company are not responsible for the outcome or negative impact our service may or may not have on your credit profile. The referring Mortgage Broker and Realty Company are not liable in any way for the results or general outcome of our service.

OUTCOME GUARANTEE

Every situation is different and because of that, our case by case basis cannot guarantee a specific outcome or accurately predict how long it will take. The dispute process does require time and persistence, so it may take more or less than six months, but this month to month agreement can be cancelled at any time.

REFUND AND WARRANTY

We do not offer refunds or warranties because our service is fully performed before you are charged for the services.

LIMITED POWER OF ATTORNEY

In order to successfully follow through with our services, we require your permission to sign, draft, mail letters and electronically submit documents to collection agencies, creditors, and credit bureaus on behalf and in your name. The Limited Power of Attorney authorizes and directs Frank Carrasco Company "the firm" to act as your disclosed or undisclosed agent when performing the services you have retained "the firm" to provide. "The firm" will address the items on your credit report that you identify as inaccurate, misleading or unverifiable. By granting Limited Power of Attorney to "the firm" you are granting authority and permission to write and send letters to collection agencies, creditors, and credit bureaus on your behalf and in your name. "The Client" also gives "the firm" permission the obtain information by phone, fax, or the internet from record holders and discuss information with record holders to negotiate a debt (when applicable). By submitting your electronic signature you are agreeing to the Limited Power of Attorney between "the firm" and "the client". You may revoke your Limited Power of Attorney at any time by emailing us at creditislife@frankcarrascocompany.com or cal1-888-636-3543.

ENTIRE AGREEMENT

This Agreement represents the entire agreement of the parties hereto with respect to the subject matter hereof, and supersedes any and all prior agreements with respect thereto. This Agreement shall not be amended or modified, except in writing signed by each of the parties hereto.

RIGHT OF CANCELLATION

You may cancel this Agreement without penalty or obligation at any time before midnight of the 5th day after the date on which you signed the contract. See the attached notice of cancellation form for an explanation of this right. You can cancel by emailing us at creditislife@frankcarrascocompany.com or call us at 1-888-6363543.

NOTICE OF CANCELLATION

Frank Carrasco Company allows you to cancel this Agreement at any time without penalty or future obligation and federal law requires that you be given two Notices of Cancellation.                                          
NOTICE OF CANCELLATION

You may cancel this Agreement, without any penalty or obligation, at any time before midnight on the 5th day after agreeing to the Agreement and Disclosures. To cancel this Agreement, mail or deliver a signed and dated copy of this cancellation notice, or any other written notice to Frank Carrasco Company; 5804 Babcock #318; San Antonio, TX 78240.
